How to Get a Medical Marijuana Card in Oklahoma

Through the approval of the SQ 788 bill, Oklahoma patients enjoyed the legalization of medical marijuana on June 26, 2018. Roughly six months after the bill was passed, accredited dispensaries were already fully and legally operational, with residents already growing cannabis plants in their own gardens. This made Oklahoma’s Medical Marijuana Program one of the fastest progressing MMJ programs in the United States during that time.

Step 1. Download And Print A Copy Of The Physician Recommendation Form
Even without a qualifying condition, you can apply for a medical marijuana card as long as a physician recommends administration of medical cannabis.
To start your application process, download a copy of the Physician Recommendation Form from the Oklahoma Medical Marijuana Authority (OMMA) website and bring this form during your medical consultation.
Step 2. Schedule A Doctor’s Appointment
Not all licensed Oklahoma physicians are registered with Oklahoma’s Medical Marijuana Program. This is nothing to worry about as registration is not required in order to issue a recommendation. Still, we want to make the process even easier for you by having one of our own doctors assist you.

Step 3. Process Your Online Application
Once you have your recommendation ready, you may start your online application. Visit the OMMA website at http://omma.ok.gov/ to register your email address and validate the email. You will then be asked to fill out the online application form and enter the following information:
- Patient’s full legal name
- Date of birth
- Permanent mailing address
- Telephone number
- Email address
- Primary physician information
Additionally, you will be required to submit the following documentation along with your application:
Proof of residency:
- Oklahoma driver’s license (front and back)
- Oklahoma identification card (front and back)
- Oklahoma voter ID card
- Utility bill (for the calendar month preceding the date of application)
- Residential property deed to an Oklahoma property
- Current rental agreement for an Oklahoma residential property
Proof of identity:
- Oklahoma driver’s license (front and back)
- Oklahoma identification card (front and back)
- A US passport or any ID issued by the US government
- A tribal ID card approved for identification purposes by the Oklahoma Department of Public Safety
Clear full-face digital photograph (for your medical marijuana card)
- Plain white or off-white background.
- No glasses or hats.
- No teeth showing.
- Top of head and top of shoulders can be seen.
Make sure to submit your Physician Recommendation Form with your online application.
Step 4. Make A Payment
After filling out the online application form, complete the process by paying the application fee.
The standard non-refundable fee is $100. Patients who can submit proof of their Medicaid or Medicare enrollment or 100% Disabled Veteran Status are qualified for the reduced cost of $20.
Note that payments must be made using a Visa, Mastercard, or Discover credit or debit card.
After making a payment, submit your online application form to be reviewed by Oklahoma Medical Marijuana Authority.
Step 5. Wait For The Card
Once your application is approved, you should receive a notification, and your medical marijuana card, in the mail within 14 days.
If OMMA finds discrepancies with your application, you will be notified via email.
Your Oklahoma medical marijuana card is valid for two years, unless revoked on grounds of federal violation.
